Luigi Mangione pleaded guilty Thursday to the murder of UnitedHealthcare CEO Brian Thompson, ending a high-profile case that shocked the American healthcare industry.

Standing in a Manhattan courtroom, the 26-year-old admitted to the December 4 shooting outside a midtown hotel. The plea deal avoids a lengthy trial, with Mangione accepting a sentence of life in prison without the possibility of parole.

The shooting sparked immediate outrage and deep-seated conversations about the American insurance system. Thompson, 50, was targeted as he walked to an investor conference. Surveillance footage captured a masked gunman approaching him from behind, firing at point-blank range, and fleeing the scene. The precision of the attack led authorities to classify it as a targeted assassination rather than a random act of violence.

For the healthcare sector, the murder exposed raw nerves. Thompson led a massive corporation often criticized for its complex prior-authorization processes and coverage denials. Following the shooting, investigators found a manifesto in Mangione’s possession. He leveled sharp accusations against the insurance industry, claiming his actions were a protest against systemic greed and the denial of patient care.

Prosecutors didn’t shy away from the underlying motives, but they remained focused on the brutality of the act. The plea brings a swift conclusion to a case that lasted less than three months from the arrest in Pennsylvania to the final conviction.

The victim’s family, who sat through the proceedings, expressed a quiet sense of relief as the judge confirmed the sentence. While the legal chapter is closed, the debate Thompson’s death ignited—about the role of corporate executives in the lives of millions of patients—shows few signs of cooling down.

“This was a calculated, cold-blooded execution,” the prosecutor told the court. “The defendant may seek to frame this as a protest, but the law recognizes it only as murder.”

Mangione offered no further statement as he was led out of the courtroom. He will be transferred to state prison to begin his life sentence immediately.
