Pillows are a normal part of bedtime for most people, but some people wonder whether sleeping without one could improve posture, reduce neck discomfort or provide other health benefits.

The answer is not the same for everyone. Whether sleeping without a pillow is helpful depends largely on your sleeping position, mattress, body shape and individual neck alignment.

Why does pillow height matter?

A pillow changes the position of the head and neck while you sleep. If it is too high or too low, it may place the neck in an uncomfortable position for several hours.

A pillow that keeps the head and neck in a relatively neutral position can help reduce unnecessary strain.

What about stomach sleepers?

People who sleep on their stomach may sometimes find a very thick pillow uncomfortable because it pushes the head upward and increases neck rotation.

For some stomach sleepers, using a very thin pillow—or occasionally no pillow—may feel more comfortable.

However, this does not mean everyone should remove their pillow.

Side sleepers may need more support

Side sleepers generally need enough pillow height to fill the space between the head and mattress and keep the neck aligned with the spine.

Sleeping without enough support may cause the head to tilt downward, potentially creating stiffness or discomfort.

Back sleepers have different needs

Back sleepers often benefit from moderate support that keeps the head from being pushed too far forward.

A pillow that is too high can make the neck bend forward, while one that is too low may not provide enough support.

Should you stop using a pillow?

Not necessarily. There is no universal rule that sleeping without a pillow is healthier.

If you wake up with neck pain, stiffness or headaches, consider whether your pillow is too high, too low or unsuitable for your sleeping position. Changing the pillow may be more useful than simply eliminating it.

The bottom line

Sleeping without a pillow may feel comfortable for some people, particularly certain stomach sleepers, but it is not automatically better for everyone.

The main goal should be comfortable sleep with the head and neck in a neutral, supported position. If you regularly experience pain during or after sleep, consider discussing persistent symptoms with a healthcare professional.
