A major scientific review has highlighted how nicotine can directly harm blood vessels and contribute to serious cardiovascular problems.

According to a major review published in the European Heart Journal, nicotine can directly damage the cardiovascular system by affecting the cells that line and control blood vessels. Researchers describe a chain of harmful biological effects in which nicotine activates receptors in vascular cells, reducing the activity of mechanisms that help blood vessels remain relaxed and functioning properly. This can promote oxidative stress, inflammation and processes linked to blood clot formation.

The review warns that nicotine’s cardiovascular effects are not limited to traditional cigarettes. Nicotine delivered through vapes, heated tobacco products, shisha and oral nicotine products can also affect the heart and blood vessels, although the overall level of harm may differ between products. The European Society of Cardiology says no nicotine-containing product is completely risk-free for cardiovascular health.

Experts say the findings provide a clearer picture of how nicotine may contribute to blood vessel dysfunction, increased blood pressure, inflammation and a higher risk of cardiovascular disease. The review adds to growing concerns about the widespread use of newer nicotine products, particularly among young people.
